Like, for instance, you fall asleep while sitting on the toilet and then just fall off the thing? I feel like I've come close a couple times, but it hasn't happened yet.

No, once I was potty trained I pretty much managed to use the toilet without incident or complication.

If the toilet seat was up and i somehow tried to sit on the rim i could absolutely fall in. Hasn't happened thankfully. Almost did once when i was living alone, my boyfriend stayed the night and left the seat up, and i tried to pee without turning on the lights.
