That might be too harsh and it might lead to Bert feeling that his anger and abusive drinking binges are justified on some level. Whatever shitty paranoid YouTube videos Bert has been consuming in his spare time have probably taught him to deal with things by pretending that any news he doesn't like is a hoax or, failing that, constructing a worldview where he is a brave martyr persecuted for thinking the truth. It would be easy for him to rationalize an aggressive therapy like ECT into this narrative where it might even drive him further over the edge.
Various CBT programs, whether using a dialectic approach or not, could be effective in his treatment. However, we should probably lay some groundwork first. It's difficult to validate someone's feelings when they genuinely can't distinguish reality from the emotional outrage-porn propaganda they have been huffing for the past few years. We really have to start establishing a some footholds for healthy emotional responses.
To this end, I suggest we use CVT. Cat Video Therapy is an approach that has been shown to work in a wide range of situations with patients of all ages. Starting Bert's therapy with cat videos has the additional benefit of teaching him to find other uses for video sharing sites on the internet. YouTube doesn't have to be a place that he visits
just to prop up his fantasy that he is morally and intellectually superior to whatever caricature of a public figure is featured in this week's two minutes of hate. The hope is that eventually he will come to see that reiterating someone else's shitty political talking points and bad faith arguments is a waste of time. While the outrage might make him feel like he has a direction and purpose for a short while, eventually he can come to accept that he has just been some propagandist's dupe.
Once we start to see some positive reaction to the cat videos and he begins to learn that YouTube is a fun place where you can experience joy by watching innocent kittens chase a ball, then we can start with some other therapeutic approaches and begin validating his feelings. After he has rediscovered some basic joy in life that doesn't come from seeking praise in vilifying other people's political enemies, then we can begin to make some progress.
