First Blood the movie took a real A-Team approach to the violence. We see car crashes, explosions, and hear sheriff's deputies screaming in pain in the woods. The stories of these people are never really resolved, and much the violence is left to the imagination of the viewer. The David Morrell novel the movie was based on is more graphic with a much higher body count.

In both the book and the movie, Rambo is triggered by poor treatment at the hands of a small town sheriff. It is through the theme of Rambo's POW flashbacks that the story explores the treatment of troops returning home from Vietnam. That Rambo can't tell the difference between his treatment at the hands of the Viet Cong and his treatment at the hands of Sheriff Teasle is seen as social commentary about how the US treats those returning from an unpopular war. Teasle is portrayed as a Korean war veteran in both the book and the movie, so his treatment of Rambo is a particularly biting remark about the lack of consideration Vietnam vets were facing from all sides.
