No, no one buys them. The 21 trillion is used to pay off all the existing bonds. The bondholders have to take it, because the bonds entitle the bondholders to receive X dollars at some point in the future. So you hold a Treasury bond entitling you to receive $X and the Treasury sends you $X. The bond is paid off. The dollars will be worth a lot less than bondholders thought they would be, but that's their fault for not reading the fine print.

People should remain well. Survival of the fittest. Homo sapiens have been around successfully for about 350,000 years. Quick look at web says health care started in Germany 1883.

Given our ancestors did sufficiently well for 348,000 years without health care. Just something to ponder.
