Gender roles aren't the same as biological sex. There are only two sexes, with some intersex people who exhibit characteristics of both. Genders are basically the socially designated role people of those sexes play in society. There are traditionally in the US two genders for adults (men for males, women for females) and the juvenile forms of those for those under 18 (boys and girls). Each person has a series of expectations and roles that don't necessarily correspond to their sex for a good reason.
Just as a banal example, it is usual for women to wear dresses and unusual for men to do so. There is no biological reason for this to be the case, but it is part of our gender norms. Rules like women can wear skirts but men can't, or women should be teachers and housekeepers while men work in factories and finance have no concrete biological basis, and have changed and will change over time. When people talk about other genders they are talking about stuff like this. They are not imagining a third kind of genitalia, but a third role for people whose genitalia have boxed them into a role they are not happy with. There is some flexibility within gender roles themselves. Women can wear pantsuits today and all that, but for people who want to be out of the traditional man/woman system, or mix elements of the system by doing both conventionally man and woman things to a socially unacceptable degree, they identify as a non-traditional/third gender.
As a percentage of the population they are like 1% but the outrage they cause is wildly disproportionate to the actual amount of influence they have to actually change anything about traditional gender roles on a grand scale.

XXY exists for sure but there are only two kinds of sex cells, sperm and egg. You produce one, the other, in rare cases both or neither, but there is no third kind of anything going on anywhere.

Can you be clearly biologically female and produce sperm? As far as I understand it there is a spectrum with Male at one end, female at the other and in the middle of the spectrum there are a large number of people who don't fit neatly into one category or the other, but there is as far as I know no affirmative "third sex".

If we identify sexes by chromosomes and there are viable humans outside of the norm then they are neither male nor female. If this was biologically impossible then why would they be alive?
