Just wondering really. Would a college allow a student to study on campus for 25 years no matter how much money they had? What about a college like Harvard? Wouldn't it damage their reputation if a student couldn't graduate?

Colleges don't care how long someone is there as long as they're paying. Colleges are businesses.

Who would know about it? Only interviewers. And so at the interview the hospital (in your example) would simply ask the candidate why he or she is now 45 years old and applying for their first job as a junior doctor. If the candidate blames the university, the interviewer can smply say: "No, that can't be it, because everyone else graduated in the normal time. More likely, you are a rich moron, yes?" Like that.

The college I finished kicks you out if you don't finish the programme in <amount of year it lasts>*2+1 years...
So for a bachelor degree that takes regularly 3 years, you have to finish it in 7 or getthefuck out. There's also a rule that you get kicked out if you fail one subject 3 times.
And I assume any sane institution has something like that (if I'm not mistaken, it's in the Bologna declaration).
Short answer: you can't study for 20 years you fucking NEET.

apparently my school doesn't really care about how long you take, you just can't keep studying forever after you go over the requisite number of credits. and even then you can stay, but any classes you take that aren't directly toward your program don't count for your gpa.

Lol, all my grades counted towards my GPA, I had like 230 ECTS at the end of my bachelors (180 needed, a.k.a. 60 per year). I'm glad they all counted, the extra ones were good grades from subjects I cared about.

community colleges would. in fact you can just audit courses for no credit. i went back a few years ago because i was bored and audited some art classes and stuff. it was fun. in fact it would probably be pretty cool to go back and audit some astronomy courses. a lot of the stuff i learned back in the day is just now really being confirmed, gravity waves, higgs, etc. so it would be neat to learn about what they're working towards over the next 10 years. taking some music theory type courses would be fun too since i quit music well before i got to theory.
anyway, if you're actually looking to be a life time student then it's totally doable, just not really with universities. Trump University, i guess, but not like a real university with like legit instructors and course work.
