Dr. Phil earned his PhD in clinical psychology and a post doctorate in forensic psychology.
You're right in the fact that he doesn't have an MD and is not a doctor in the literal sense, but people with doctorate degrees are given the honorary title of doctor, eg (Doctor of Philosophy, Doctor of Management, Doctor of Engineering, etc)
