Very analytical question, a difficult question. I've never given it much thought but I think they are biblical insults. Like:

Helvítis - hell
Andskotans - Satan's
Djöfulsins - another word for satan

Then there is "hálfviti" which means half-witt

Otherwise I think we just use English words.

I read somewhere the Dutch have disease based insults, and they tell you to lick their ass instead of kiss it. Do Icelanders have insults unique to them?

Lol lick their ass? I can think of one insult that may be unique to icelandic, but even then it's sort of a funny rather than a harmful insult:

shit-walker (Kukkalabbi)

It means someone who doesn't wipe his ass, but this is largely unknown and mostly used by angry toddlers.
