No, she cannot. There was an episode where Giles asked her to spot vampires in the Bronze. Giles was all serious about how the slayer should be able to sense them in her very being, but she spotted one because his outfit was out of date and looked like a disco era outfit. So as long as you acted and dressed normal, she would not know you were a vampire.

There was a not shown on TV pilot with worse clothing and makeup, a less blond Buffy, and an uglier fatter Willow.
I had gotten mine from eBay because YouTube didn't exist then, but it's on there now.
