Most major cities would have been wiped out, and man would be set back by the loss of these assets to humanity, but time would bring more cities.
Even today there are not enough nuclear bombs to wipe everyone off the planet.
Chances are some country would have come out ahead, and stayed in their place. Their culture becoming the dominant one, and eliminating any future nuclear holocausts.

Taking into account fallout, and other global effects still doesn't wipe out everyone. It kills a lot of people, and causes tons of other shit (birth defects, cancer, blindness, etc) but doesn't wipe everyone out. There are still many people that would walk away from nuclear war alive.
