EA added new gender customization options which means the original option for male or female is "gender identity" as this determines the pronouns and presentation for that specific sim.

However, biological sex characteristics are now in the game too. You can now select rather or not your sim has "masculine" or "feminine" body frame; their preferred style of dress, with both options the same; rather they can become pregnant, impregnate others, or neither; and rather or not they can pee standing up.

This was one of my genderfluid characters, Banu Sultan, who is currently assigned as 'male' but I could at any point re-edit him and assign him to be a her using the new "Change Sim" interaction on all mirrors and dressers.
