You really shouldn't discuss what communism would allow and wouldn't allow socially when this is an epistemic impossibility. Do you think pre-agricultural societies could envision a mode of production in which private property prevails? Do you think pre-capitalist societies would envision a mode of production operated purely on the basis of exchange?
However, we know what communism is the NEGATION of - but you clearly don't. The implication implicit in this post is that a communist society continues to be dominated by the law of value. This is utterly absurd; communism is the resolution of the contradiction between use and exchange. Production under communism, as it is planned by society as opposed to being dominated by the logic of capital accumulation, occurs on the basis of what society finds useful. Hence Marx's slogan of "from each according to their ability, to each according to their need". This quite naturally ensures access to medical care for all.
