It may have started in the 90s, but I've only seen it starting in the early 2000s. Since 2010, I have seen this to be quite a common thing which I also don't understand.

I think it may have started in part to give context to images to make them more exciting, particularly to put the subject or viewer in fetish-related scenarios. For example, a picture of a naked girl is just a naked girl. But a naked girl with the caption "pleas put down the plunger, big brother" is an erotic incest fantasy.
