I couldn't see this guy's nub directly, but I knew it was there and flapping, because his shirt kept flapping here and there. It was very distracting but I felt it was rude to look at his flapping nub area.

No, I agree that doesn't make sense. That would be more like a pat on the shoulder than a handshake. I guess this guy would have to shake with his left hand or not shake at all. If I lost my right arm, I would just not shake at all.
