That was because the board doesn't lock tables or use transactions when creating replies which causes a race condition. Two users for the first time in a thread at exactly the same time have a chance of being assigned the same poster number. This happens very rarely, but nonetheless is terrible design.

How finely is time resolved for this? To the second, 1/10 second, etc? I've never seen it here before so it is a special thing to be sure.

It's not about time resolution, it happens when two requests are being processed concurrently by the server and they reach that code at the same time.

Yes, but in a grotesque way. The repositories for the server minichan is on didn't see an update yet, and honestly the entire server is crap. It's from some shady vps provider we got a few year ago, currently moving to another server.
