The Iranian government's "solution" for homosexuality is to endorse, and fully pay for, sex reassignment surgery.[18] The leader of Iran's Islamic Revolution, Ayatollah Ruhollah Khomeini, issued a fatwa declaring sex reassignment surgery permissible for "diagnosed transsexuals."[18] Eshaghian's documentary, Be Like Others, chronicles a number of stories of Iranian gay men who feel transitioning is the only way to avoid further persecution, jail and/or execution.[18] The head of Iran's main transsexual organization, Maryam Khatoon Molkara—who convinced Khomeini to issue the fatwa on transsexuality—confirmed that some people who undergo operations are gay rather than transsexual.[19]

My issue is not that they transition because they're gay. My problem is that they transition because their hormones are going wacky during puberty.

That's a good question. I'm not sure what the laws are on it, and googling around didn't reveal anything.

Personally, I'd say it's a very complex issue. On paper, something like 16 or 18 sounds reasonable. They'd probably be mature enough at that stage to make a decision. Problem is, children can feel uncomfortable as their birth gender so much earlier than that. In a documentary I watched, a child with GID actually tried to cut off his own penis because he didn't feel that it belonged on his body.

In regards to that, I'd say the reasonable solution to this would be to let them live as the opposite gender until around 16 without actually transitioning, and then give them the option of transitioning. That way they're making the decision as mature young adults, and are probably certain that it's what they want. Just a rough idea on it, I'm not a doctor who understands the procedure or anything. If somebody with more knowledge cares to weigh in, I'd be happy to hear from you.
