I work with/for/on/ One of the key players in Sat biz. All Satellite services END up connection to the Ordinary Internet. Gateways to the same net you and I use.
Exception is of course DOD. DOD decided for protection to build their own Direct to Satellites (Centers) So sure they also use the regular Net systems but and this is only a guess (not really) They also have private links that do not use the net you and I use.
I use for work a private service that is 100% free of the internet - Uses ordinary T1 connections to the phone company. Two Paths just in case one local phone CO (Ops center) goes down - Usually that is because some dirt digger cuts a simple buried cable.

Yea in fact that is exactly what Iridium does. Motorola when they designed that beast did not want to deal with country by country permissions for ground stations. So all links are Sat to Sat. Huge failure because of on board processing and all the stuff that makes them heavy as all hell and the hand offs Sat to Sat - Bottom line is they got it to work butttttt with super poor sound and almost no data capability - First year of net wayyyyy back had a faster dial up data rate.
But your concept of doing it all without a ground station that could end up --taken down --- well keep in mind that there is a massive delay for GEO's so cell in the sky that way fails and to put a full net in space - Hey leave that up to the Si-Fi writers.
