I usually bite random spots. When the icecream looks uneven, I lick it counter clockwise or clockwise till its all even and smooth again.

I kind of push the icecream as I go so that I don't have empty cone left at the end. A little icecream gets on my top lip, but that's what the napkins come with them for.
