don't use the copyright complaint form on youtube never works
just send an email to [email protected], you can use fake names and stuff and it won't matter, they never check nor do they care, they figure if you take the time to file a DMCA or have the knowledge you must be legit (a DMCA claim takes less then 5 minutes to write lol)
if the other person counter dmcas, just make a new email use another set of fake info and repeat, if you keep doing this youtube will just get pissed and suspend the persons account cause of too many claims

you probably don't have to, it may be only for google takedown requests (as per their DMCA policy), but you may as well put it and try to make it look as legit as possible upon casual viewing.
they're only going to barely glance at the information other than what to remove, but having it there makes it seem more legit, y'know?

yeah, you should read the policy of whatever site you are requesting a takedown on, just make sure you include the information they require and anything else is unnecessary.
you're supposed to follow the guidelines layed out in the actual DMCA too, but since your claim would fall apart if it got that far anyway, that would be superfluous.

You don't need a YouTube account to send them a DMCA notice and you don't need to use their gay webform either.
Don't send anything to "[email protected]" like someone suggested, it doesn't exist. Send emails to [email protected], or [email protected]. A list of things they want you to include in your notice is here. You can ignore the phone number and scanned signature part, they never complain if you don't include them anyway.
And don't use stupid names or anything, because requests are reviewed, and they're not stupid.
