As someone who wasted almost a decade unhappy I can tell you it is a BIG mistake to waste your time unhappy. Of course you want it to work out etc, and in a perfect-world with perfect people it would work out, but in reality life is short and in the long run it probably won't work out. Even if it does work out you're still not gonna be happy, you know she won't really change... regardless of what she may say. You can be grateful for her taking care of your grandmother, but that isn't a reason to stay with her. Ask yourself if you have more reasons to split than to stay together, if you have more reasons to split, than there's no need to waste everyone's time. Breaking up is hard to do, but its almost always best for everyone.

My thoughts: for me, the bottom line in a relationship is this: "Are you happy?" If not, then what is the point of suffering to prop up a relationship that is not working? You can be grateful for her care of your grandmother, and tell her that, but that does not mean that you have to be involved with her romantically.

Sounds like its already over, you just haven't told her yet. You shouldn't feel guilty for leaving her. She didn't take care of her grandmother as part of a contract that you could never leave her.
