You must not understand Trans* people at all. Let's learn:
gender = what your mind says you are
sex = what your body is
For most people, these two align. Head says boy, body is boy. But for some people, it gets mixed up. Their mind says they're a girl, but their body isn't. It's extremely difficult to understand, and very difficult for the person to go through. That's why people get operations like that, to get the two in check.

I forget which part of the brain it is, but they found a really tiny area, in the human brain that differs between males and females. Could be that the boy in the OP ended up with the female brain part instead of the male one somehow.

Let me ask a hypothetical question. What if it was discovered that gay/transgender feelings were the result of a hormonal imbalance that could be fixed with a single pill?

I don't see the problem really. It sounds like she finds her life much more enjoyable/bearable as a girl, so why not? It's better to start transitions earlier (pre-puberty) to avoid having to undo the wrong puberty changes. If it was the girl's idea, then she will at least know that it was her choice.
