It is intended as a question, just not one warranting a lengthy or particularly genuine response. If someone asks you "What's up?" and yesterday you won the lottery, your entire family hijacked two jet airplanes and crashed them into the World Trade Center, and you won the Nobel Peace Prize, then you would say something like "life's been busy lately, what's up with you?". You can elaborate more if they ask follow-up questions but otherwise you briefly answer their question and ask an equally meaningless one back to them before you start having the actual conversation you actually want to have. This is so basic wtf.
